Freezing Cream Soups

I have a wonderful cream of chicken soup that I want to take from Philadelphia to my father in North Carolina. He is very ill and it is his favorite soup.

1. Can it be frozen without affecting the quality?
2. If not, if it is kept cold, is it still safe to consume after 5 days?
